Does insurance cover the costs?
We are in network with the following insurers: Allcare, CareOregon (Jackson Care Connect), Moda, Regence BCBS, and Providence. If you are covered by another insurance company (out of network), we offer a flat cash payment rate ($1,300) and provide you with a superbill that some insurance companies may reimburse. As the insured member, you are the best person to determine your coverage. We will check your insurance coverage as a courtesy. Costs not covered by your insurance are expected to be paid at your first appointment.
What does our ADHD evaluation look like? What activities should I expect?
We gather evidence from a variety of sources in order to differentiate poor regulation of attention from various causes or conditions. This includes developmental and family history, interviews, standardized rating scales, direct neuropsychological testing, and observations made during the interview and assessment techniques designed to illicit characteristics of executive functioning and memory.
The evaluation includes the following procedures:
-
Intake forms with developmental History
-
Records review (as appropriate)
-
Clinical Interview
-
Intellectual (IQ) Brief Measure
-
Dyslexia Screen (if necessary)
-
Mental Health Screening including depression and anxiety
-
Standardized, ADHD specific rating scales
-
Standardized scales of daily executive functioning skills
-
Neuropsychological testing (e.g., auditory and visual working memory, selective and sustained attention, planning/organization, task monitoring, cognitive flexibility, impulse control, processing speed, continuous performance measures, etc.)
-
Psychological report with diagnosis and intervention recommendations
-
Referrals to other providers/school teams as needed/requested
What is the process?
Step 1
Request an evaluation through our website, email, or phone – You can request your own appointment, or we accept referrals from your PCP or behavioral health provider.
Step 2
A licensed therapist (LCP, LCSW) will schedule your initial appointment (in person or telehealth) and send you a link to the patient portal to complete the intake forms.
Step 3
The therapist will complete a diagnostic interview and initial screening. If the screening is consistent with an attention disorder, the therapist will schedule your comprehensive evaluation with Dr. Fry.
Step 4
Dr. Fry completes his neuropsychological assessment, writes a report, and reviews results with you then refers you back to the therapist for treatment planning as appropriate. Of course, you are also welcome at this point to seek services from your current provider, another therapist, or choose not to have behavioral-based treatment. Even if your care with us ends, you will still have everything in the care package - membership to Chadd, books, psychological evaluation report, and other resources.
Should you choose to receive ADHD-related therapy, you will work with a private therapist who will develop a targeted treatment plan and bill for their on-going services:
Step 5
Your therapist develops an individualized treatment plan to include a prescribed number of sessions of psychotherapy (e.g., Cognitive Behavioral Therapy for ADHD, neuropsychotherapy, etc.). As part of your plan, you will have access to Cogmed, a computer assisted CBT (cCBT) to improve working memory and attention. Note: the use of Cogmed is complementary/adjunct treatment to your regular therapy and is included in your treatment plan at no cost.
Step 6
Use Cogmed for memory and attention training in the comfort of your own home on a computer, tablet, or smartphone during the week, then attend a weekly session with your therapist to work on your CBT-related treatment goals and discuss on your progress with Cogmed
