You will be expected to pay for each appointment at the time it is held, unless we agree otherwise.  Payment may in the form of cash, check, or credit/debit card (Visa, Mastercard, American Express, and Discover).  If I cannot bill your insurance for evaluation costs, I will provide you with a superbill, which is an itemized receipt for services, that you may submit to your insurance company for possible reimbursement, dependent on your insurance plan. This means that you are responsible for the full fee for services at the time services are rendered. You may also wish to check with your employer for information regarding your flexible spending account (FSA).  I will provide you with a receipt for all services rendered.  I accept the following insurance as an in-network provider:  Regence/BCBSProvidence, Moda, Allcare, and JCC. I can bill Oregon Health Plan (OHP) if you are eligible at the time of service and the procedure is covered by your plan.

Consultation appointments (90 mins) are billed at a flat rate of $300.  My hourly fee is $200.  In addition to scheduled appointments, I charge this amount for other professional services you may need, though I will break down the hourly cost if I work for periods less than one hour.  Other services include, test scoring and interpretation, report writing, telephone conversations lasting more than 10 minutes, attendance of meetings with other professionals you have authorized, site visits, travel time, preparation of records or treatment summaries, and the time spent performing any other services you may request of me.

Comprehensive Neuropsychological Evaluation

This type of evaluation typically includes:

  • Review of records

  • Clinical Interview

  • Direct testing of neuropsycholgical functioning (Intelligence, attention and executive functioning, memory, language, motor, learning, social, academic, and behavior).

  • Observation

  • Standardized behavior rating scales

  • Scoring and interpretation of results

  • Written report with diagnosis and treatment recommendations

  • Post evaluation meeting

Psychological/Psychoeducational Evaluation

This type of evaluation typically includes:

  • Records review

  • Clinical interview for psychological diagnosis

  • Personality assessment

  • Specific test batteries depending on presenting problems

  • Rating scales and inventories

  • Scoring and interpretation of results

  • Written report

  • Post evaluation meeting

Specific tests and procedures can be tailored to address your individual needs.  Here are a few examples:

  • Cognitive strengths and weakness profile

  • Intelligence testing for giftedness

  • Academic functioning

  • Social/emotional/behavior functioning

  • Career/vocational/post-secondary educational planning

Consultation, Training, and Other Services

These services are billed at $300 for the initial meeting and $200/Hour thereafter as needed.  These include such services as:

  • Consulting on mental health-related concerns

  • Treatment Planning

  • Professional Workshops

  • IEP/school evaluation report review

  • Letter of support with recommendations
